Disability Case Management & Rehabilitation Planning
Disability Case Management services begin with a functionally focused review of all relevant case information. This includes a thorough examination of medical and rehabilitation records, a clear understanding of critical occupational demands, and an in-depth initial assessment with the client. This assessment can be conducted in person, virtually via secure video conferencing, or over the telephone, and aims to gather comprehensive insights into the client's current medical and functional status, treatment plan, and barriers to wellness and return to work.

Following this review, a proactive rehabilitation plan is developed and implemented with the goal of achieving a sustainable and successful return to work.
These services are designed to support individuals in preparing for and planning their return to pre-disability or pre-injury roles, modified positions, or alternate employment. Tailored to each client's specific needs, the services can include:

Developing a gradual or modified return-to-work plan in collaboration with the individual, their treatment providers/physicians, employer, insurers, and other relevant stakeholders.

Facilitating a return-to-work planning meeting with the individual and their employer representative.

Implementing and monitoring the return-to-work plan to ensure a safe, successful, and sustainable reintegration into the workforce.
